Community Healthcare System is now Powers Health19Paent ExperiencePaent & Family Advisory Council provide a voice in careCommunity Healthcare System nurses play a key role in developing and maintaining strong Paent & Family Advisory Council (PFAC). PFAC oers paents and families the opportunity to partner and provide guidance on how to improve the paent-family experience. The goal of the PFAC is to create a more paent-centered approach to the care delivered every day. Engaging paents has shown to improve the quality and safety of the care delivered, lower the costs of care and reduce medical error. Our healthcare teams are commied to creang an environment where the needs of paents and families drive the healthcare experience. Through our Paent and Family Advisory Councils, our hospitals welcome paents or family members to join us and share their perspecves.PFAC members do not need any special qualicaons to be an adviser; however, they or their family members must have received services at the hospital within the last three years. Through their unique perspecve, advisers help shape changes to improve hospital quality and safety and enhance the overall paent and family experience at our hospitals. MyChart BedsidePaents at the hospitals of Community Healthcare System use an innovave app called MyChart Bedside as an added connecon to their care while in the hospital. By downloading the app to their personal tablets or phones, paents can use it to review menus, daily rounes, lab results, vital signs and informaon regarding their care team. Once at home, their MyChart Bedside informaon can be transferred to their regular MyChart account.Messages from paents can help improve care by allowing sta to address the paents’ concerns. The system oers a new level of communicaon that enhances care and encourages paents to stay involved in their care aer leaving the hospital.0% 10% 20% 30% 40% 50% 60% 70% 80%HCAHPS Inpatient Units:Nurse Always Communicated Well80%74%79%78%Indiana Average National AverageCommunity Stroke &Rehabilitation Center **from NRC patient experience dataCommunity HospitalSt. CatherineHospitalSt. Mary Medical Center
